Service agreement

VERSION 2026-10-05
WEALTHYLIFE BUSINESS CONSULTING — SERVICE AGREEMENT
Version: 2026-10-05-v1

1. SCOPE AND START OF SERVICE
The client engages WealthyLife Business Consulting for the services selected in this agreement. No service work begins before the authorized client representative signs, applicable payment is received, required records and authorizations are provided, and the office confirms the service start. A portal account, inquiry, or payment alone does not begin services. Services apply only where required for the client's operation.

2. MONTHLY MANAGEMENT
Compliance Plus: $249/month, up to 3 drivers. Compliance Management: $399/month, up to 5 drivers. Fleets of 6 or more drivers require a separate written quote. Monthly support includes routine deadline monitoring, reminders, document/file maintenance, missing-document follow-up, and quarterly file reviews. It does not include unlimited repairs or automatically include the cost of individual filings, testing, government fees, or other separately priced services. A change in fleet size requires a confirmed plan adjustment. Initial complete trucking business and compliance setup is a $1,250 professional service fee when selected; official and third-party charges are separate.

3. FEES AND PAYMENT
Selected individual services follow the published WealthyLife price sheet and fee breakdowns. Items marked varies, custom, starting price, or and up require a written final quote before work starts. Government, agency, tax, registration, insurance, permits, licensing, laboratory, and third-party charges not specifically included are paid by the client. Monthly fees are due at the beginning of each service month. Payment is processed through the supplied GoDaddy payment link or invoice. Signing this agreement does not authorize automatic card charges; any recurring billing authorization is completed separately with the payment provider. WealthyLife records payment only after verifying it with the provider.

4. PROJECT WORK
DataQ challenges, audits or extensive audit preparation/responses, corrective actions, authority applications/changes, reinstatements, court matters, and extensive file reconstruction are separately scoped and billed. WealthyLife will provide a separate quote and obtain client authorization before work beyond the selected scope.

5. ESTIMATED TURNAROUND AND WAIT TIMES
Business days exclude weekends and holidays. Estimates begin only when the signed agreement, applicable payment, complete documents, and required authorization have been received. Routine letters, MCS-150, UCR, BOC-3 and MVR requests: 2–5 business days. Driver/vehicle files, binders and handbooks: 5–10 business days. Business startup, USDOT and authority preparation/submission: 5–10 business days. IFTA, IRP and Form 2290 preparation: 3–7 business days. Clearinghouse/Motus assistance and reinstatement administrative work: 3–7 business days. Monthly onboarding: 5–10 business days once initial files and tracking dates are ready. Drug testing and inspections depend on confirmed appointment availability; laboratory results vary. DataQ, audits, court matters and major projects receive a written project estimate. These are WealthyLife planning estimates, not guaranteed completion dates. Government reviews, approval periods, systems, postal delivery, laboratories, and third-party processing add time. Missing or incorrect information pauses the estimate. An approaching deadline does not guarantee expedited service. The office will communicate a revised estimate when material delays are known.

6. CLIENT RESPONSIBILITIES AND DEADLINES
The client provides accurate, current records and promptly reports changes in drivers, vehicles, contact information, coverage, registrations, operations, and expiration dates. The client obtains required signatures, consents, designations, and authorizations for MVR, Clearinghouse, and testing services. Do not upload account passwords or unnecessary Social Security numbers. Monitoring relies on the dates and records supplied and reviewed. The client remains responsible for safe operations, legal requirements, required filings, and timely action. Reminders do not guarantee delivery, approval, or avoidance of penalties. WealthyLife does not promise that any filing, challenge, reinstatement, or application will be approved.

7. ALERTS AND RECORDS
The client consents to service-related in-portal and email reminders to the agreed contact addresses. Reminder targets are 60, 30, 14, and 7 days before a recorded due date, plus overdue follow-up. External email delivery begins only when the notification service is connected. Text alerts require separate setup and consent. Documents are accessible to the assigned client and authorized office users. Records are used to provide the selected services and shared with agencies or necessary service providers only as required or authorized. The client should retain original records and report access or contact changes promptly.

8. CANCELLATION AND SERVICE HOLDS
Monthly services continue month to month until canceled by either party with 30 days' written notice. Payment for the current service period and completed or authorized work remains due. A cancellation does not reverse government/third-party fees already paid or work already performed. Unused prepaid service amounts will be reviewed and reconciled based on work completed and fees incurred. Services may be paused for missing documents, lack of required authorization, or unpaid balances after notice. The office will identify outstanding items and explain the impact on timelines.

9. ACKNOWLEDGMENT AND ELECTRONIC SIGNATURE
The signer confirms authority to bind the named client company, has reviewed the selected services, prices, fee responsibilities, scope, and wait times, and consents to an electronic signature and electronic copy. The typed full name, signer's role, account email, selected services, exact terms version, and timestamp are saved together. This administrative agreement does not create legal representation; legal matters require separately retained counsel where needed. Changes to scope or pricing must be documented and accepted in writing. Questions: compliance@wealthylifebusinessconsulting.com | 800-507-2505.
